Downloading is simple, just go to https://translations.launchpad.net/ubuntu/jaunty/+source/$package/+pots/$package/+export and click Request Download, then one should dump the contents of the resulting arcive to $package/po and make an upload.
I think we should also compile a short list list of applications that should undergo this procedure, so that none of them will be forgotten, while keeping the overhead low - e.g. just to translate some GConf schema descriptions, I wouldn't want to harass packagers.
