My Apologies, in retrospect I have a completely unrelated problem - my LCD was connected via one of the DVI connectors on my graphics card (I am trying to install ubuntu for the first time here) and I got
a) Good b) Good Then nothing more, so presumably c) and d) should have followed. I found a DVI to VGA converter thing and used that to plug my monitor in and can now see c) and d) and have clicked on the install button.
