I attempted to reproduce this issue on Ubuntu 18.04 (Bionic), which is
the last Ubuntu release where chm2pdf is available.

I created a minimal CHM using Microsoft HTML Help Workshop with image
paths differing only by case (e.g. Images/test.png vs images/test.png).
The CHM renders correctly on Windows (the image is displayed).

On Bionic, chm2pdf fails to handle this input. Both normal conversion
(--webpage) and --extract-only crash with:

TypeError: cannot concatenate 'str' and 'NoneType' objects
(originating from Python 2 sgmllib).
