I think the current structure of the team is very vague and we could do
with defining things a bit better. Currently decisions are made by
discussing things until we can reach a consensus. When this works I
think it is the best way to work but we are eventually going to come up
against a decision where we can't reach a consensus and then we need to
have some alternative. 

I'm in favor of keeping the team as loose as possible but would like to
define who has a vote if we can't reach a consensus.

I like this idea. Are there any objections or can we get a volunteer to
look after it and link to it from the main team wiki page?

I'm not too keen on initiatives requiring approval from anyone. If there
is someone willing to do something do they really need to get approval?
I
think that is only required where you want other people to do something
(though it is still good to discuss all plans).
