The CVD doc conversion found multiple problems with figures. Some of these
were traced to incorrect CSS and XSL-FO customization layers. Fix these to
properly center tables and figures.
Also careful checking showed line space slightly off - change from 1.4 to 1.35.
Also - specify the width parameter on the CVD Jpegs so the width for html is
the same as specifying no-widths; this causes the PDFs to be reasonably sized.
