Hi Paul, I believe they do connect off-net, I helped some that I believe used a non-BT Fibre connection in the Bury St Edmunds area a couple of years back and they had a couple of the Cisco SPA's but of course they may have changed firewalls etc.
I believe the soft clients connect to a slightly different platform/SBC, so BT may have started firewalling.
