Ok, so it is launched but somehow the server dies. Look into /var/log/squeezeboxserver/server.log the reason may be there.
Also, I suppose you have no use for mysql besides squeezebox server. Did you try a reinstall of mysql-server and mysql-client ? There is a chance the database files went corrupt.
