Mhh. This is the trace of squeezeboxserver_safe doing its relaunching thing. Obviously the server itself does not kick in at all.
Perhaps you can try "sudo /etc/init.d/squeezeboxserver stop" to stop the relaunch process, then do manually what squeezeboxserver_safe is doing: launch the server. I hope you will see something on the screen when it goes away. I would try this: "sudo -u squeezeboxserver /usr/sbin/squeezeboxserver --diag" (You can kill the server -in case it does not die...- by hitting control-c in the window, once you're done.) This should launch mysql as well.
