I am (sometimes) using Sugarcube with CSOS and it is working fine (I
just checked).  My setup is similar, with MusicIP running on a Windows 7
machine and Sugarcube using Dynamic Path Conversion very much like
yours.

First thing I would do is to set logging to 'debug' for plugin.sugarcube
on the LMS > Settings > Advanced > Logging page.  
Then try playing a few individual tracks and check the LMS server log
file.  

I usually find that the log gives some very good indicators as to what
is going wrong with sugarcube. 

Other, maybe obvious, things to check is that the LMS License Manager is
running OK (check that Sugarcube settings page for your Player is
showing "License Status: License is Valid") and check that there hasn't
been a change / update to the Windows Firewall that might have started
blocking access.
