I have been doing some investigation and have the following thoughts:
Is it possible that HiFiBerry have changed the DAC that they use on
their boards? I say this because their datasheet for the DAC+ Pro
states the maximum rate supported is 192kHz/24bit, however in the
Squeezelite log file the maximum rate reported by the board is 384kHz:

In the HiFiBerry forum, there is a report of a problem that I thought
might be related to my problem but not quite the same:

The reason I thought it wasn't the same was because the reported problem
only appears after playing a lower sample rate track and mine is
consistent and doesn't require playing a lower bit rate track before
trying to play a 192/24 track. However I have seen in the logs that
when Squeezelite opens the DAC, it specifies a sample rate of 44.1:

and therefore, even if the first track played is 192/24, the card will
have to switch sample rates from 44.1 to 192 and because there are two
clocks on the Pro card, it will also have to switch clocks. The
HifiBerry team have been able to reproduce the problem and state:

I don't think clock switching can be the whole story, as I am able to
play 24/96 tracks that would also require a clock switch.
I have been having problems signing up to the HiFiBerry forum but I will
post my issue on that thread, once I can find a way to sign up (Firefox
prevents me doing it because it thinks there is a security issue, so I
will try another browser).
