Hello I have the same problem as Michael, using JDK 1.4.2_04 on Win XP SP2. This is also a random error.
I also tried to close every possible program accessing the document (browser tabs in firefox, whole browser firefox/ie, xml editors, even my antivirus software), it still does randomly crash. This is under the bundled Jetty, I do not have enough time yet to try this under Tomcat. This is quite embarrassing, because when this error happens, the document switches into some ghost state, i.e all its menus options are greyed, and I can't get rid of it anymore, not even by restarting the container. Unfortunately I don't have the luxury to use a linux box just to "fix" this error.
