I'm sure this is way off base, but the problems I had last week which were
similar to yours (random 404 errors which go away--sometimes--on refresh) turned
out to be that I was running MySQL as root (which I knew was a bad idea--but
thought it was only a security issue).   The problems went away when I started
MySQL from Webmin as another user (admin, in this case).  I don't know enough
about it to quite understand why, except that root seemed to be allowed only one
connection.  I found out when I tried to log in on Webmin as root from two
machines at the same time, but was refused on the second.

Of course, as soon as I send this, I'll probably start getting 404's again!
That's the problem with random errors...  ;^)

there aren't any frames they are sections in the layout of the page that
load the page together with a number of functions snippets.

Unfortunately I can't replicate the error because it happens randomly I
guess

Is this a bug of Midgard 1.4, the database was originally siting on a 1.25
system and it worked fine. It was when It was modified
to work on a new system bases on 1.4 that this error started appearing.

I am using a number of servers with both RedHat 6.1, RedHat 7.0, Mandrake
7.2, Apache 1.3.12, Apache 1.3.19

I get the 404 on all of them randomly when I refresh the page appears

I even increased the max_connections

If it happens tomorrow I will send you the error log again
