in /etc/my.cnf (location may differ of course):

[mysqld]
#log=/var/lib/mysql/mysql.log
#port   = 3306

uncomment or add the log-line and restart mysql server.

So, I'm back at mod_midgard not being activated. Can you share the bit
of httpd.conf that has the AddModule and LoadModule directives for
Midgard with us?

btw, I can't reach you phpinfo() output from here.
