Do we talk about the form navigation bar which you can show/hide via
View/Toolbars, or do you have a dedicated *form control* in your
document which is a toolbar?
The latter does not contain a search icon, the former does.
If you're sure it's about the former, then try customizing it
("Customize toolbar" in the drop down menu which appears when you click
on the down-arrow at the right hand side of the toolbar).
If "Find Record" doesn't appear in the customization dialog, then have a
look at the file
<office_installation>/share/config/soffice.cfg/modules/swriter/toolbar/formsnavigationbar.xml.
Does it contain a line with ".uno:RecSearch"?
