In fact there is no need for the image to be the full width of the page. It
works for smaller images if you choose the Position (Left Middle in my
case). If your image is the same height as the Header Height e.g. 0.5cm or
49 pixels at 300dpi the image won't show completely. Increasing the Header
Height to 0.8cm solves the problem. But this means that if you really need
the header to be 0.5cm you need to compensate...

I can see the image in the Page Preview screen.
