This is reproducible in LibreOffice 3.6.6.2 (Build ID: f969faf) and 4.0.4.2 (Build ID: 9e9821abd0ffdbc09cd8c52eaa574fa09eb08f2).

Really puzzling. Any ideas?

I don't find this particularly puzzling: you have two merged cells across four rows of your table, not two cells in two rows. The only reason you would merge cells instead of simply enlarging them is that you want to retain the separate cells in other columns. The rows are still there and are still taken account of in numbering.

Workarounds?

Yes:
o Number the rows manually.
o Create a table of two rows; nest other tables of one column and two rows each in the cells of the right column. o Create separate tables for each column with appropriate numbers and heights of rows. You may be helped by frames or by multiple columns in your page or in a section.
