I would suspect that you have corrupted the CSV file by using a 'wordprocessor' to edit the data. You should be using a spreadsheet program that is familiar with CSV files or even a simple text editor. It would seem that the channel name fields have been relocated to a field that requires an integer value. Probably time to start over.
