Thanks Doug for a very clear and complete description of how to create a new
"Graphics Styles" element. But I was asking about creating "Presentation
Styles". Can you explain that one with as much detail?

I can only confirm the following in OOo 1.1.4 and 1.9.71

Assuming those of you struggling with this topic have figured out how to 
  open the Stylist, using OOo 1.1.4 at the very top you will see one of 
two possible titles:  "Graphics Styles" or "Presentation Styles".  If 
you see "Presentatation Styles" then (1) you have *_not_* followed 
instructions to "Select the icon on the left of bar", and (2) you need a 
refresher course about the meanings of Left and Right, or (3) your 
dyslexia has not been properly diagnosed.

Using OOo 1.9.71, there is only one title, "Styles and Formatting," 
which reduces the number of cues that you're straying so you will have 
to be extra careful about selecting the correct icon.

Ensure that the combo box at the bottom of the Stylist window is showing 
"All Styles".  If it doesn't, then make that selection.  If you have 
selected the correct icon at the top, then the first entry in the list 
in the Main box of the Stylist will be "Default" and the last will be 
"Title2".  (I know this is true because you haven't been able to get far 
enough to create any custom styles.) Place your mouse cursor over any 
one of these list items.  Press and release the RIGHT mouse button, or 
see (2) or (3) above.  You will now see a context menu that contains the 
entries "New..." and "Modify..."  If you do not release the mouse 
button, nothing will be displayed, so you must follow the instructions.

Lastly, if you place the mouse cursor anywhere in the blank area *below* 
the list of styles and right click, the context menu will contain only 
one choice - "New..."
