[users] Creating a form - connected to database

For a mystery shopping research I am trying to create a scaled-response 
form that looks something like this:
Criterium---------Poor (1) - Fair (2) - Good (3) - Very Good (4) 
Excellent (5).
Instead of using the words every time, I would like to create a field 
with 5 option boxes. When an option box is selected by the user, this 
should create an input in the database, dependent of the option box 
checked, of a 1, 2, 3, 4 or a 5.
I have read the document "How to link tables using named parameters", 
but this did not show me how to do it.
Is there anybody who can tell me how to do this? I use 1.1.4
