OK, found it. It was the Language Tool Spell checker add-on version 0.9.4. I installed this while writing some articles for Linux Identity on OOo.

There is a note that claims that there is a bug in OOo that causes some items to crash. The underlying API in OOo is new and to my knowledge, still under development. I know for certain (meaning I was told by developers) that it was not yet finished when the 3.0 code was frozen.
