If you are using Windows, the kind of problem you describe is often the result of an earlier fault that left an apparently, but not actually, active OOo session running. To clear things up without restarting, you can bring up the Task Manager with Ctrl-Alt-Del, and look under Processes for soffice.bin.End any such process (which will also end any associated soffice.exe processes), and OOo should come up again OK.

As for bug reports -- this is definitely a non-trivial process, but in general this list is a good place to start when you think you've found one. Chances are very good that it either is not a bug, or it has already been reported, and you're likely to get that information here.

Interesting that you mention this because I was experiencing s similar problem just an hour before your post wherein Writer wouldn't open any .rtf file sent back from our "cloud" app, even with Ctrl held down. When I looked at Task Mgr there were multiple (like 4-6) copies of swriter as well as soffice.exe and soffice.bin. Only after I killed all of them and opened one copy to be minimized did Writer operate appropriately.

If they are already loaded why do additional copies get loaded?

I think the key is to get rid of the soffice.bin processes. I'm not sure, but it seems the soffice.exe processes are "children" of soffice.bin (they vanish when you close the soffice.bin processes). There's probably a separate swriter.exe for each open text document, not sure if they're children of soffice.exe or not but I've never been stuck after closing all of the soffice.bin processes (Quickstarter may have one, but I never have that going).
