What do you mean by creating headers on the fly?
I am creating headers by writing line of text, selecting it and clicking
"header 1" (or 2) from menu available after clicking F11.

I have created TOC when some of the document was already present, but it was
modified later. Some of the headers were deleted or modified, some were
added. TOC worked fine with OO 1.1.4.
Additional note:
I've found a way to make OO 1.1.4 fail with TOC.
a) Get "test.sxw" from url I've posted a few mails ago.
b) Load it using OO 1.1.4, update TOC and verify it's OK.
c) Load the file using OO 2.0.
d) Update TOC - it is incorrect then.
e) Save the file as test2.sxw (yes, in 1.0 format).
f) Load test2.sxw using OO 1.1.4.
g) Update TOC using OO 1.1.4 - it is _incorrect_ now. Save changes.
I hope somebody who knows internals of sxw format could understand what's
the problem from differencies beetween test.sxw and test2.sxw after those
operations.
