There is a mistake on

A question reads:
1) Is this the first time you are completing an OpenOffice.org registration
survey ?
[*] Yes - this is the first time I take the registration survey.
[*] ...
/// The above is incorrect grammar.
The first suggested response should properly read:
[*]Yes - this is the first time I've taken the registration survey.
or
[*]Yes - I'm taking the registration survey for the first time.
//
The question is also awkward.
It should properly read:
1) Is this the first time you've taken an OpenOffice.org registration survey
?
or
1) Is this the first time you've completed an OpenOffice.org registration
survey?
or
1) Have you previously completed an OpenOffice.org registration survey?
