Pls try this option:
Choose View|Toolbar|Controls
On that toolbar click the second icon, to tun on design mode
Now click on the fifth icon, Check box.
Drag one in a cell of your chooise
Change the format of the checkbox, and the label
(rightclick, Contol for window with properties)
If you are hapy with it, you can copy the contol as much as you want.
Don't forget to turn of designmode.
