I use 2.0.2 on both Windows and Linux with the same results. The Layer
spinner sets the maximum outline level that will be used rather than the
level used.
Both the header and footer seem to do the same thing: search for the
last maximum outline level that exists prior to it. For the header, this
is the previous page. (On Page 1, this is the first such header.) For the
footer, this is the last such header on the same page.
For example, I wanted to have both a Heading 1 and Heading 2 in the
header. For page 1, the results was Heading 1 and then Heading 1. Page 2
on, it was Heading 1 and then Heading 2.
