Did your computer go into hibernate mode between opening and saving such a file or did you load your file from a network drive that possibly lost connection at least for a moment? Or do you have a tool running that cleans the "temp" folder while OOo or the OOo quickstarter is still running? These are the most common root causes for the effect you described.
