In the US, it used to be standard.   That was when US$30 for 30 hours
per month, or US$60 for 90 hours per month were "bargain rates". for
Internet access.

Across the board, I'd say that the level oof net neutrality back then
was about the same as it today --- a few stupid firms (Comcast, AOL)
actively preventing people from using the Internet in a manner that
meets the consumers needs, but does meet the needs of the telcos that
should be permanently barred from doing anything but providing a
pipeline.
