Zen? Newnet? To name just two. I can (and have) done just that - paid in advance for extra. (I am on Zen.) I have clients on Newnet. They allow either payment in advance or payment when the moment comes - but the second is at a higher rate. It is cheaper for me to maintain a "pool" of prepaid extra bandwidth than to to go to the next package up. There are also many other ISPs who use the ad hoc system.
