Right, I rebuilt a custom integrator, disabling the checkExtensionsUpdate call, and this has resulted in working extensiosn for one of these problem users, so I can confirm this is the same issue as reported by OP.

Have you also tried for those users to disable checkExtensionsUpdate() ? You could try so, rezip the XPI and send them directly for testing purposes. It'll give us more beef one what could be the issue.
