In progressive Marin County, California, many years ago the local small distance signs began including km distances in addition to miles. These are smallish vertical white signs about 60 cm x 15 cm placed at odd intervals mainly for use by road workers and police personnel to report on road repairs and traffic accidents. They are not replaced often, and I think most of these are still in place. I don't know about the design of new ones.
