That is an excellent web site reference you provided.  I added it to my favorites list to refer to again at a future time.
 
Actually, according the Jacobs website, Jacobs manufactures and sells both types of brakes that we have been discussing.  The engine brake you described, which uses some mysterious (to me) hydraulic device, takes over control of the exhaust valves and, as you said, is not attached to the exhaust system.  That system is offered for Class 7 and 8 truck engines which are the large, 18 wheeler, over the road trucks.
 
If you go back to the Jacobs web site and scan down the page, you will find the exhaust brakes we have been talking about.  The one Jacobs manufactures if offered for the Cummins powered Dodge Ram.  It is installed in the exhaust system and operates by blocking release of exhaust gasses.  That is also the same basic design as B-D and other light truck exhaust brake manufacturers use for light truck and RV applications.

The Jake Brake isn’t an exhaust brake, it’s an engine brake.  The exhaust system remains open.  The Jake Brake introduces a high volume of air into the cylinders to create back pressure, and I would guess that the loud rapping noise is produced by the sudden release of that air each time the exhaust valves open.   

Jacobs does make an exhaust brake, which is called  the Extarder:

I would guess that true exhaust brakes are a lot quieter than Jake Brakes (?).

 I would have thought blocking the exhaust would make the diesel quieter... can someone straighten my convoluted understanding of this brake system??
