We have 5 patients we receiving IVIG regularly and have administered
IVIG in the home setting for a little over 3 years.  We have an IVIG
policy which identifies the assessment data, what the initial orders
must contain and the standard precautions to follow in identifying the
most appropriate infusion process.  The script must be reviewed by the
pharmacist and the patient must have received the dose in a controlled
setting without ADR.  In addition to the regular visit note, the nurse
completes an IVIG infusion flow sheet with titration parameters, v/s
schedule in relationship to the rate changes.  
Now, we have had an incident where a pt had a reaction with a dose (had
been receiving IVIG at home for 2 years), reminding us not to become
complacent with our routine infusions and that any pt can have a
reaction any time with any med we are administering.  But, we followed
our ADR protocol and it was resolved without further problems.
