I have always given 50 mL of D50 by hand injection through
whatever catheter the patient had. The pharmacy provided this as a
prefilled syringe so there was nothing that needed to be done before
administration. I have never given it by infusion.
The rationale from your pharmacy does not make sense to me. More
dilution produces a greater chance in osmolarity. So putting this in a
500 mL bag of D5W would cause a greater change in the final osmolarity
than putting it in a 50 mL bag. So I am not understanding your

We have the d50 put into a 50cc bag of D5W and run it as fast as possible via a pump.
Now pharmacy is telling us it needs to go into a 500cc bag in order not to change the osmolarity.
What are the rest of you doing out there. We need your input.
