I agree with Lynn.   We have always given D50 via whatever size catheter the 
patient has and have done it as an IV push.   Granted D50  can be hard to push 
but have never really encountered any problems hand pushing.  In our 
institution D50 has always come in a prefilled syringe and is readily available 
to give to a patient when needed.

I have always given 50 mL of D50 by hand injection through whatever catheter 
the patient had. The pharmacy provided this as a prefilled syringe so there was 
nothing that needed to be done before administration. I have never given it by 
infusion.

The rationale from your pharmacy does not make sense to me. More dilution 
produces a greater chance in osmolarity. So putting this in a 500 mL bag of D5W 
would cause a greater change in the final osmolarity than putting it in a 50 mL 
bag. So I am not understanding your question. Lynn
