Our policy is to call the physician if the platelet count is less than 20,000, and make sure they don't want to give some platelets before the PICC insertion. I don't actually this low platelet count often myself. If the doc didn't want to give platelets first (when this has happened, they have always ordered the platelets), I would go ahead and insert the PICC.

A PICC is the safer of all central lines to place with low plt ct for the reason Lynn stated.What we do at our facility is ask the MD for an order for a unit of plt to be given just prior to PICC insertion and we don't nick prior to inserting the introducer.
