You sure it didn't happen before this?  Any chance that you simply

Okay, so far so good.

All the *-02 plexes are disconnected here.  They are disabled because
there is no storage associated with them.  In addition, they are not
part of any volume.  You could simply delete them (with vxedit).

And here you've shown a working volume, but instead of a -01 and -02
plex, you have a -01 and a -03 plex.

My assumption is that someone has changed out a drive (c1t0d0) in the
past, and moved the VxVM objects by hand or something.  As part of the
process the old (empty) *-02 plexes were left around.

I doubt the patches had anything to do with this.

I don't see anything wrong as a first pass other than some leftover
objects that should probably be deleted.
