Unfortunately I can't help you there, and we have to hope that a
MacVim person will explain how you configure an ftplugin script
(I monitor vim_mac only because I am a manager who moderates the
first message from new users to prevent spam).

Assuming you have ':syntax on' somewhere, you can always try the
script by simply running it when you are editing a csv file:
    :e example.csv
    :so csv.vim

The above assumes example.csv and csv.vim are in your current
directory; add paths if not.

I just tried the above after starting Vim with -u NONE and it
was pretty disappointing because the color scheme was hopeless.
My defaults show much clearer highlighting.
