Let me clear this out: MacVim = gvim for Mac.  It handles ftplugins
just like on any other platform (and I myself have files in
.vim/ftplugin that load just fine).  I can only guess that the OP's
problem has something to do with other plugins or rc-file settings
interfering.

Etienne: have you tried moving .vim/.vimrc/.gvimrc out of the way and
creating a new empty .vim folder, then adding only the necessary files
for csv.vim (see [1])?
