I don't know why editing over sshfs is slow. The reason why the key repeat is slower in the GUI is probably because MacVim drops repeated keys when Vim can't keep up with processing them. You can try asking on vim_use as to what causes the slowness when editing over sshfs. There may be some option to disable to make things quicker (backup files, etc.). I seem to recall that this has been discussed there before. I will put this on my todo list and try to investigate further when I get some time, but I don't think there is much I can do about this.
