Thanks to everyone. In addition to your suggestions I've implemented a
couple of other changes on my WinXP Thinkpad as a result of googling
for help on this.

One is that I used a registry fix to swap my Ctrl keys with my Alt
keys so now the Ctrl function is adjacent to the space bar and I can
hit it with my thumb.  This change helps more in emacs, but it also
help in using the Ctrl-[ option for vim.  See:

Another is that I used Microsoft Keyboard Layout Creator to swap
parentheses with square brackets so that parens are now easier to get
to.  (I think I could have achieved this in the same registry fix as
above.)
