No, a SCSI terminator is not required in this simple of a configuration. My first guess would be jumpers. It can be any SCSI ID except 7 but it is usually 0. Typically 0 is programmed by not putting in any jumpers in the SCSI ID positions. Other jumpers vary so you might want to post what they are here.
When you say it's an official Apple drive I take it you mean it says "Apple" on the drive, if not it probably isn't. You could try modifying a copy of a bootable floppy by adding some SCSI tools to it. I'd suggest a SCSI scanning tool and a non-Apple disk format tool. Do you have access to another Mac with a SCSI interface?
