In my opinion, it is not related to missing or deleted software. It is more likely a result of age (wear and tear) on the diskette drive.
One thing that I have seen happen over time is that the diskette drive continues to work fine but only for diskettes which are formatted by that one diskette drive. So you can save files off your Mac to diskettes and load them back later from the diskettes fine, but if you try to use a diskette that was formatted on another machine, you may get the error you have been seeing where it can't read the diskette and asks you if you want to format it. I think this just means that there's a problem with the alignment of the read/write head on the diskette drive. You could try using a diskette drive cleaner but in most cases this kind of fault indicates it is time to replace the diskette drive.
I have always been able to find replacement diskette drives on ebay for not too much money, although with the age of your IIci you might find you have to spend as much on the diskette drive as you paid for the whole computer!!
