I don't know about getting them on to the iPhone, but I did find a very 
accessible converter program, for those of you who have a set of mp3 
ringtones you want to use on your iPhone.
it is not free, but in my view, well worth the money, as you get the full 
file, none of this messing around with having to select bits of a file to 
convert, or a 30 second only limit rubbish.
this can be found at:

and it will cope with fairly large batches of files, it worked just fine 
when I chucked 162 files at it for example.

here are my instructions for NVDA:
I have used NVDA 12.2 and Copytrans manager 0.978
to add music and RingTones  to my iPhone without installing iTunes.

This is the best free program to manage iPhone iPod or iPad without
installing iTunes.
Connect iPhone to PC.

Start CopyTrans Manager, If it does not lose focus,
NVDA should announce Main Tab.
wait about 12 seconds to allow the program to connect to the iPhone.
 From the main tab, press the Tab key 4 times to focus on the list of
media such as audio ringtones & books.
NVDA may speak the first item on this list only.
using the arrow keys will move the high lighted item, but will not speak it.
dont worry about it. just press the applications key
and up arrow to Add, it is the second option from the bottom.
this should open a window allowing you to browse for music to add to
your iPhone, iPod or iPad.
This window should act as a normal browse window.
so select the files you wish to add and enter on them.
now back on the list of files that NVDA want speak, just exit the
program, you will be asked to save changes.
Thats all. its not perfect, but its better than nothing.
