For me I seem to have the volume issues when I have audioducking off. As soon as I turn it on the problem goes away.
I'm going to email accessibility at apple. The only trouble is I don't know how to pinpoint when the problem occurs, only how to resolve it. The trouble is I don't like audioducking, but I seem to be stuck with it.
