I too have the issue with audio ducking turned off. I mostly have audio ducking off anyway, turning it on only when I absolutely have to hear speech. Most of the time I can hear and mostly understand both VoiceOver speech and another form of audio. This depends on the audio though, such as loud music. Anyway, it is hard to tell when the volume is going to go bonkers. Yesterday I had turned my VoiceOver volume down low at work, but hwen I locked my phone, the phone lock sound seemed to be much louder than VO speech.
