Logic tells me that the more you have running in the background, the less you can work with, because it is being used elsewhere. With that in mind, I always empty the app switcher of apps I am not currently using.

You really never need to close apps, especially with iOS7 or 8 and the newer iPhones or iPads. The system manages things well, and makes apps sleep or work in the most efficient way it can. The only time you'd need to close an app is if it is misbehaving (sucking up data or power like crazy, or not responding correctly). Mostly, you can leave apps alone and you'll be fine. The App Switcher is a quick way to access recent apps, or apps being handed off by other Apple devices, but it isn't necessary to keep it cleaned out. It isn't that the focus is on the wrong app, it's that VoiceOver is silent, not speaking the app at all. Simply touch the screen to focus; you'll have to do that anyway, to close it, or to start swiping to other apps, so it's really nothing extra.
