I tested calling myself from a different line to see if when I’m on the home 
page all would work as easily as if I’d called from a contact or the phone app. 
It does. The truth seems to e that sometimes the phone is less responsive and 
predictable than at other times.

                I realize that the keypad won’t come up until the phone is 
answered at the other end or a message machine picks up. Once again, sometimes 
the keypad is spoken immediately and other times it is not. Likewise, the 
double tap does not often work to end a call nor answer a call.

                Are my gestures imprecise? Sometimes they might be but overall 
I can operate the phone quite precisely. 

            I’m using a 6.

A few observations:

First of all you don't have to find the answer button when the phone rings. You 
can do a 2 finger double-tap. Of course, if you want the other options you have 
to find the buttons after all.

 

 

Also, when you make a call, the keypad does not appear until the line is 
answered, I think. So, while it's ringing, there's no keypad but, ones there's 
something on the other end of the line, you should see a keypad as long as vo 
is on. 

If this does not work I'm not shure what else is going on.

I'm not following what you mean by the Phone App. Whether that app is open or 
not, a keypad will always appear during a call unless you do something to 
dismiss it (lock the device or hit the Home Button, for instance). I also 
notice no lag when using VoiceOver during a call, and I'm on an iPhone 5. I 
don't remember what device you have, but perhaps it is an older one?
