Can you clarify where you find exactly the problem happens, I read this thread and find it unclear. You say in a couple messages that you find it happens more on earphones than with the speakerphone. Do you mean actually it happens more if the Apple Earpods (the headset which comes with your iPhone is plugged in) or do you mean it happens more when the phone is held up against your ear and the sound is coming from what would be the earpiece on a regular phone, e.g. the speaker that is built into the phone and which is against your ear? If so, have you tried to use it with an actual headset? It might be better if you test it with Voiceover turned off altogether. Without Voiceover calls do not switch automatically between external speaker and internal speaker. You can turn off Voiceover while you are on a call as long as you have set the triple click home to turn off Voiceover. If it is happening more with a wired headset connected, then I would say the problem is maybe the headset itself.
