Lisa, When your iPhone has focus in iTunes, have you set it to transfer purchases so that anything downloaded to the phone will transfer into your computer? Also, with focus on your device, have you navigated to the Tones button/tab and made sure that the Sync Tones checkbox is checked and the radio button of all tones is selected?

I know there was a problem previously with Itunes, and i've restored my ringtones a few times. I have IOS 8.2 and the latest version of Itunes, and the problem persists. It seems as though it happens when I sync my phone to my computer. The last time I tried to restore ringtones it said that I had already done it, but my ringtones aren't on my phone.
