When you turn off speech with the gesture you will still have voiceover running 
in the background. This means that all the normal voiceover gestures will still 
apply. Once you turn off voice over with the triple click home jester you will 
lose all of the normal voiceover gestures and the phone will go back to normal 
operation. It is helpful sometimes to turn off speech when running apps that 
have their own voice built in. Also if you were watching a video or listening 
to a podcast you might want to turn off speech to avoid getting interrupted by 
different alerts and such.
